Preventative Services Coordinator-

JOB SUMMARY:

The Preventative Services Coordinator is responsible for recruiting, screening, training, and supervising volunteer mentors to work with children as specified by the current grant under which the Preventative Services Program operates. This position will also facilitate weekly support groups at school sites, recreational activities, and service learning opportunities. Additionally, the coordinator will collect data and compile reports to the appropriate funding agencies This position is generally a part-time, non-exempt position which does not include medical, dental or vision benefits and does not accrue vacation, sick or holiday leave in accordance with the agency’s Personnel Policy and Procedures.

QUALIFICATIONS, EDUCATION & EXPERIENCE

An undergraduate degree is required.
One year of experience in a related field is required; two years’ experience is preferred.
Relevant experience may be substituted for the required education, as deemed appropriate by the Executive Director.
Special consideration will be given to persons with knowledge of community-based organizations.
Special consideration will be given to persons who speak and understand Spanish.

RESPONSIBILITIES & DUTIES

Receive referrals, conduct mentee interviews, assess needs, and determine appropriateness to the program
Conduct ongoing mentor recruitment, screening, and training.
Conduct ongoing mentee recruitment, engagement and matching.
Match Mentors and Mentees together effectively.
Lead groups at various school and community sites when appropriate.
Document client progress, establish outcome goals, maintain files, and terminate cases according to established procedures.
Maintain accurate client statistics of both mentors and mentees.
Maintain appropriate client referral service linkages; make appropriate referrals for mentee services when necessary.
Monitor the Mentor/Mentee matches through in person, phone and email contact.
Conduct monthly meetings with Mentors and initiate strategies to retain mentors.
Work with the leadership team to accomplish goals of the program.
Perform a quarterly review with all match participants, assessing outcomes and establishing new outcomes, as appropriate.
Perform Match Closure interviews to measure overall results.
Maintain required program statistical data. Document progress through quarterly and annual reports.
Ensure continuity of funding by assisting the Executive Director with proposals to funding sources.
Develop resources within the community to serve children and families.
Participate in regularly scheduled staff meetings and related functions.
Assist in planning and carrying out of agency fundraising activities.
Answer the telephone and greet clients and public professionally, pleasantly and courteously.
Acquire updated information through reading and training to remain current on job related techniques, issues and information.
Maintain accurate and timely records.
Participate in appropriate networking activities.
Share in office coverage and maintenance when necessary.
Perform all tasks and assignments to ensure appropriate and ethical adherence to the grant/contract guidelines under which the program operates.
Share in office cleaning and maintenance duties as requested.
Other responsibilities, as assigned, to support department/business needs.
Perform all related tasks as requested by the Executive Director.
